2018 Salute to Outstanding Women Nomination Form

YWCA Mohawk Valley will honor an outstanding woman from each of the categories listed below and, new this year, one female youth. An independent panel of judges has the responsibility of evaluating and selecting the award recipients. Selections are based solely on the information provided on this nomination form.

Eligibility: Nominees must be 18 years of age or older, live or work in Oneida or Herkimer County and must have demonstrated excellence and leadership in the workplace and/or the community. Outstanding youth nominee must be 17 years of age or younger and live, work, or attend school in Oneida or Herkimer County. Nominees cannot be past Salute honorees, current YWCA MV Board members, or current YWCA MV staff.

How to Nominate: Nomination forms may be faxed to 315.732.8760, emailed to events@ywcamv.org, or mailed to: YWCA Mohawk Valley Attention Salute Nomination, 7 Rutger Park, Utica, NY 13501. Please do not send additional materials beyond responses to the questions on this form. Additional documentation will not be considered.

Nomination Deadline: Close of Business Friday, February 16, 2018

  • Education: Teachers, instructors, administrators, community educators, coaches, mentors, professors, librarians, tutors, etc.
  • *Entrepreneur: Individuals who have started their own business, large or small, store front or online, etc.
  • Healthcare: Physicians, nurses, healthcare professionals, opticians, dentists, therapists, health aides, pharmacists, etc.
  • Human and Public Service: Law enforcement, fire department, armed forces, EMT, social workers, non-profit employees, etc.
  • *Outstanding Youth: Young women, age 17 or under, who deserve recognition for their school-related or academic achievements, and/or volunteer and community contributions.
  • Professions/Business & Industry: Employees of all levels in any profession, business or industry. Includes, but is not limited to, formal education, training, or skills – attorneys, accountants, construction, marketing, etc.
  • Social Justice: Individual who works to open the doors of access and opportunity for everyone, particularly those in greatest need.
  • STEAM: Individuals working in and/or promoting science, technology, engineering, art, or mathematics in any field.
  • Unsung Heroine: An individual who works continuously and tirelessly behind the scenes, goes the extra mile with little thanks, or has made an outstanding contribution to their community.
